Making a referral to Qualcare

The length of time taken to commence services will depend on the type of referral, individual needs of the client and the availability of staff.  Our Intake and Response Coordinator can provide you with more information about this.

Coordinating Your Services

Prior to commencing services a Service Coordinator works in partnership with the client to ensure that services are flexibly tailored to meet individual needs and lifestyle choices. A care plan is developed which is located at the client’s home so that they have a copy of it and our care team can view it when they need to.

The Service Coordinator will review the program regularly and ensure that the program changes with the changing needs of the client.  This includes visiting the clients home at least annually to make sure that the working environment and equipment is safe for our clients and disability support workers.

Flexible and Self Rostering

Clients can have as much or as little involvement in the management of their program as they choose to.  Our team is ready to take care of all the coordination and rostering needs of our clients so that they don’t have to worry about making calls and notifying people of changes.

Many clients prefer to have the contact details of their care team so that they can easily make changes to their roster and gain maximum flexibility. These clients can still contact their Service Coordinator if they need a hand.

Choosing Your Support Team

All new candidates are interviewed, reference checked and police checked before we introduce them to clients.  Once we are satisfied that they are suitable candidates we arrange a “Meet and Greet” where the client has the opportunity to ask and questions they may have. 

The client makes the final decision about who will be employed to work with them. 

Training is then provided to new candidates including shadow shifts to ensure that they understand the client’s individual requirements and routine.

After Hours Support

Qualcare is a 24 hour service.  Our after hours service can provide emergency shift cover to our clients outside of business hours.
